Abstract
Polycystic Ovary Syndrome (PCOS) is a leading cause of infertility, primarily due to anovulation caused by hormonal imbalances, insulin resistance, and disrupted ovarian function. This paper describes a 28-year-old woman with PCOS who presented with subfertility after failing to conceive for 10 months. Initial treatment involved Metformin for insulin resistance, vitamin D, folic acid, and Dydrogesterone for cycle regulation. Ovulation induction with Letrozole was initiated but showed a suboptimal response in three cycles. Success was achieved in the fourth cycle by combining Letrozole with injectable Menotropins (HMG), leading to improved follicular growth, endometrial thickness, and successful conception. This case highlights the importance of individualized ovulation induction strategies and addressing insulin resistance to optimize fertility outcomes in women with PCOS.
