Abstract
Background: The successful transition from hospital to home, in medically complex patients, requires extensive discharge education and readiness for hospital discharge (RHD). Family caregivers play a crucial role in the continuity of comprehensive care and the recovery process of medicine patients after discharge. Hence, it is essential that they consider themselves, adequately prepared and ready for this transition. However, limited studies have been conducted in developing countries regarding this phenomenon.
Purpose: The study aimed to assess the quality of discharge teaching and readiness for hospital discharge among the family caregivers of medicine patients hospitalized at a tertiary care hospital, Karachi, Pakistan.
Methodology: A quantitative cross-sectional analytical design was used to address the study question. The consecutive sampling technique was used to recruit a total of 160 family caregivers of internal medicine patients, to assess the quality of discharge teaching and the readiness for hospital discharge (RHD). The data was collected from the study participants through structured questionnaires, by using the quality of discharge teaching scale (QTDS), and the readiness for hospital discharge scale (RHDS). Moreover, the relationship of demographics, hospitalization, and clinical characteristics with family caregivers’ RHD was assessed by using one-way ANOVA, independent t-test, and Pearson correlation. Findings: The findings of the study revealed that 89.4% of the family caregivers reported RHD (>7) and the average mean score of RHDS was found to be 8.23 ± 1.07. The participants perceived a high readiness in the subscales of caregivers’ personal status (9.26 ± 0.97), followed by, expected support (8.92 ± 1.47), coping ability (8.79 ± 1.17), and knowledge status (7.87 ± 1.39); whereas, low readiness was found in the subscale of patients’ personal status. Moreover, the average mean score of QTDS was reported to be 6.81 ± 1.24, with high and low scores in the delivery skills and content received dimensions, respectively. Furthermore, the univariate analysis of independent variables showed that there was a statistically significant relationship of patients’ age (r= -0.200, p< 0.05), length of hospital stay (r= -.190, p< 0.05), functional status (F3, 156 = 8.101, p< 0.05), number of take-home medications (r= -.160, p< 0.05), and quality of discharge teaching (r= .538, p< 0.001) with family caregivers’ RHD.
Conclusion: The results of the study concluded that it is crucial to assess the quality of discharge teaching and RHD before discharge. The Meleis Transition Model served as a guiding framework for the study to determine the essential concepts in the transition process and to identify the relationship among transition variables. Moreover, the low quality of discharge teaching reported by family caregivers suggests that there is a need to focus on effective discharge planning and individualized discharge guidance in the institution to promote safe transition.
