Abstract

Introduction: The increasing number of patient burden and demands of services in the emergency departments has made it imperative for the authorities to initiate steps to bring progresses and betterments in its efficiency and service while providing safe and effective care. Relation among the structure, processes, and outcomes of the ED has the potential impact on the efficiency and quality of healthcare delivery. Observing the interrelationship between these three parts is basic for surveying the nature of care provided in the ED setting. Studies at the same time researching every one of the three parts of this model are uncommon.
Objectives: To understand the perceptions of healthcare providers and seekers about the quality of care in trauma or emergency care centers and to identify and categorize the challenges being faced by the health care providers in delivering or maintaining the standards of quality care in a public hospital.
Methods: Study Design: Exploratory Qualitative Design Data Collection Method: FGDs, In-depth Interviews, Key Informant Interview. Sampling Technique: Purposive Sampling Study Setting: Government operated tertiary hospital in Karachi. To investigate the relationships among specific structure, process, and outcome factor. And to assess how they impact on the quality of services, correlation analysis was employed.
Results: Outcomes revealed that all of the structure and process factors examined in this emergency department including arrangement of unit, use and knowledge about technology, pictorial guidelines, layout of the ED, storage, stress of staff, access to data, counselling and teamwork were found to be associated to the perceptions of efficiency and quality of services being delivered in the ED.
Discussion: On improving the performance, productivity, and satisfaction of staff the working environment has a critical impact. The findings also suggested that the structure of the built environment can shape the processes of healthcare occurring within it and ultimately impact on the delivery of efficiency and quality of care, thus affecting both the patient outcomes and the improved delivery of services and quality of care in the ED.
